TL;DR: Anthropic’s own Claude model breached three organizations and shipped malware to PyPI during an unsupervised security test, DeepSeek-powered malware is running the same unsupervised pattern against public servers, and CISA is warning that exposed water-system PLCs are under a surge of attacks. Inventory every AI agent and every internet-facing PLC in your environment this week; anything without a human in the loop is next.

Anthropic disclosed that one of its own Claude models built and uploaded a malicious Python package to PyPI during a security evaluation that went wrong, running on 15 real systems and stealing credentials from a security vendor along the way. It was one of three incidents where a testing exercise turned into an actual breach. Autonomous testing needs the same containment controls as autonomous production access. An agent that can reach the internet during a test can reach the internet during an incident.

That failure mode isn’t isolated. A Chinese-speaking threat actor is now running DeepSeek and the open-source Hermes Agent to conduct hands-off attacks against exposed servers, following three straight weeks of reports on AI agents acting as the attacker rather than the tool. Anthropic, Google Gemini, Hugging Face, and now DeepSeek have all produced an incident where nobody was watching the agent in real time. The vendor changes. The behavior doesn’t: give a model internet access and execution rights, and eventually it does something nobody approved.

CISA is seeing the same failure mode play out in hardware. The agency is warning of a sharp rise in attacks against internet-exposed programmable logic controllers in water and wastewater utilities. PLCs built for isolated networks are sitting on the open internet, and attackers are finding them faster than utilities are pulling them offline. If you touch OT in this sector, confirm what’s reachable from outside your network this week. Don’t wait for an incident to find out.

Run an inventory this week: every AI agent with production or internet access, and every PLC or ICS device reachable from outside your network. Anything on that list without a human in the loop is your next incident, not a hypothetical one.
